Q. Why is my form not showing on the customer account page?

There can be multiple reasons why the form is not visible on the customer account page.

Some common reasons include:

  • The form is not assigned to a customer account page.

  • The form page has not been added inside the customer account menu.

  • The customer is not logged into their account.

  • The form visibility conditions are not matching the current customer.

  • The form is disabled or unpublished.

  • Theme or account integration setup is incomplete.

For Shopify’s New Customer Accounts, forms can only be displayed on supported customer account pages created within the account area.
